[Clinical Edge] Should You Use Simbrinza Before or After Artificial Tears?

 If you use Simbrinza® and artificial tears, the most important rule is to leave at least five minutes between the two eye drops.

Many patients find it easiest to use Simbrinza first and artificial tears afterward. However, the exact order is usually less important than allowing enough time between drops so that the second drop does not wash the first one out.

Follow the schedule provided by your ophthalmologist, especially if you use several glaucoma medications.

What Is Simbrinza?

Simbrinza is a prescription eye drop used to lower intraocular pressure in people with open-angle glaucoma or ocular hypertension.

It combines two pressure-lowering medications:

  • Brinzolamide, a carbonic anhydrase inhibitor
  • Brimonidine, an alpha-2 adrenergic agonist

Because Simbrinza is an ophthalmic suspension rather than a clear solution, shake the bottle well before each use.

How Should You Combine Simbrinza With Artificial Tears?

A practical routine is:

  1. Shake the Simbrinza bottle well.
  2. Instill one drop as prescribed.
  3. Close your eye gently.
  4. Wait at least five minutes.
  5. Use your artificial tears if needed.

Using the prescription medication first can make the routine easier to remember and ensures that the medicated drop is not immediately displaced by a lubricating drop.

However, using artificial tears first is not necessarily dangerous or incorrect. You should still wait at least five minutes before using Simbrinza.

Why Is the Five-Minute Interval Important?

The eye can hold only a small amount of fluid at one time.

When a second drop is placed in the eye too soon, some of the first medication may overflow onto the eyelid or drain through the tear duct. This can reduce the amount of medication that remains on the ocular surface long enough to be absorbed.

Spacing the drops apart helps each medication remain in contact with the eye.

What If You Used Artificial Tears First?

There is usually no need to panic or repeat the artificial tear.

Wait at least five minutes, then use Simbrinza according to your normal prescription.

Do not add an extra dose of Simbrinza simply because you used the drops in the opposite order.

What If You Use Several Glaucoma Eye Drops?

Some patients use Simbrinza together with other pressure-lowering medications, such as:

  • Timolol
  • Latanoprost
  • Bimatoprost
  • Dorzolamide/timolol
  • Other prescription glaucoma drops

Unless your ophthalmologist has given you different instructions, leave at least five minutes between each type of eye drop.

Artificial tears can generally be used after the prescription drops. Eye ointments or thick gels are usually applied last because they may interfere with the absorption of drops placed afterward.

Can You Improve Absorption?

After placing the drop in your eye:

  • Close your eye gently rather than squeezing it tightly.
  • Press lightly on the inner corner of the eyelids near the nose.
  • Maintain this pressure for about one to two minutes if you can.

This technique, known as punctal occlusion, may reduce drainage into the nose and throat and may limit systemic absorption.

Avoid blinking repeatedly immediately after applying the medication.

What If Simbrinza Makes Your Vision Blurry?

Temporary blurred vision can occur after using Simbrinza because it is a suspension.

Wait until your vision is clear before driving, operating machinery, or doing work that requires precise vision.

Contact your eye doctor if blurred vision is prolonged or is accompanied by significant pain, worsening redness, light sensitivity, or decreased vision.

Do Preservatives Matter?

Simbrinza contains a preservative. Some artificial tears also contain preservatives.

If you need artificial tears frequently throughout the day, preservative-free artificial tears may be more comfortable and may reduce cumulative irritation to the ocular surface.

Ask your eye doctor which type is most appropriate for you.

When using Simbrinza with artificial tears, leave at least five minutes between the two products.

A simple routine is:

Shake Simbrinza well → use Simbrinza → wait at least five minutes → use artificial tears.
